Description of CME Course

A 72-year-old woman with metastatic, well-differentiated, small intestinal neuroendocrine tumor presents with worsening fatigue and flushing. Over the course of 5 years, she had undergone peptide receptor radionuclide therapy for a total of 5 treatments. Laboratory workup is significant for a hemoglobin of 7.5 g/dL, platelet count of 54?×103/µL, and absolute neutrophil count of 1310/µL. Examination of a bone marrow biopsy specimen revealed the presence of tumor cells staining strongly positive by immunohistochemistry for synaptophysin and chromogranin. What is your diagnosis?
