Description of CME Course

Chronic care management (CCM) includes patient care coordination services such as care planning, care management, help with transitions of care, and support with achieving health goals while prioritizing continuity of care. The Centers for Medicare and Medicaid Services (CMS) requires consent to be obtained for CCM services. This activity provides clarification on the frequency at which physicians and other qualified clinicians must obtain consent for these services.
