Description of CME Course
The purpose of this ECHO is to support primary care providers and their care teams in 1) preventing chronic disease through evidence-based nutrition strategies and 2) developing a deeper understanding of the social and environmental drivers of health related to food and nutrition. Educational Objectives
1. Describe evidence-based nutrition strategies for preventing and managing chronic diseases such as type
2 diabetes, cardiovascular disease, and cognitive decline
2. Demonstrate the ability to integrate brief, effective nutrition counseling into routine primary care visits using validated screening tools (e.g., Hunger Vital Sign) and motivational interviewing techniques
3. Implement dietary interventions for patients with prediabetes or metabolic syndrome and coordinating care with interdisciplinary teams
4. Understand how to make effective referrals by accurately identifying when a patient would benefit from specialized dietary counseling or support services, and when in-clinic management is sufficient
