Educational Objectives
1. Implement expert-recommended neurocognitive assessments and emerging diagnostic tools, including blood-based biomarkers, into clinical workflows to enhance early and accurate identification of cognitive decline in aging patients
2. Engage aging patients and their caregivers in personalized discussions about cognitive health, preservation strategies, and baseline assessments about modifiable risk factors for cognitive decline, fostering informed choices and collaborative care planning
3. Develop personalized, multidisciplinary care plans for patients at risk for or diagnosed with Alzheimer’s disease, targeting modifiable risk factors, based on evidence-based strategies to preserve cognitive function and delay progression
