1. Apply evidence-informed strategies, including emerging blood-based biomarkers, to support early detection and assessment of cognitive impairment in aging patients
2. Initiate compassionate, patient-centered conversations about cognitive health with patients during routine wellness and chronic disease management visits, including consideration of age-related concerns
3. Design a patient-specific management plan to address modifiable risk factors for cognitive decline during patient visits, using a holistic, evidence-informed approach that considers age, lifestyle, medical, and psychosocial determinants
