Description of CME Course

This educational activity on advanced diagnostic bronchoscopy is designed to address educational needs identified through an analysis of knowledge and practice gaps in this area. Specific topics identified include the following:
• Diagnosis and staging of lung cancer with advanced diagnostic bronchoscopy
• Principles and practice of convex probe-EBUS
• Guided bronchoscopy
• Lung cancer screening
• Mediastinal anatomy and lymph node review
• Specimen acquisition, rapid on-site evaluation, and genotyping
• Advanced diagnostic bronchoscopy techniques
• Airway management during bronchoscopy
• Transbronchial cryobiopsy use

Presenters of these topics will focus on both a review and an analysis of its best use in clinical practice. Practitioners have come to rely on this meeting to learn about the most current clinical and research studies, hear experts’ analyses of cutting-edge technical innovations, and receive guidance on the optimal use of bronchoscopy in their clinical practices.

Additional Gaps
Technical skills. Although advanced bronchoscopy techniques have potential upside for improved outcomes, the procedures are technically challenging and require specialized equipment and training. New equipment and procedural techniques are regularly being developed, which adds to the challenge for practitioners to utilize these developments to achieve optimal outcomes. This creates gaps in some practitioners’ skills and competencies to optimally perform these techniques, which could have an impact on outcomes.

Studies have shown that a significant learning curve exists for laparoscopic surgery, a similar technique, because it requires a high degree of knowledge, dexterity, and technical skills.2-5 An initial training period that includes intraoperative techniques with oversight from expert surgical advisors is recommended for surgeons to become proficient in these complex procedures. Given that bronchoscopy requires similar skills and competencies,6 and data have shown a learning curve exists for EBUS-TBNA as well,7 it is reasonable to assume that bronchoscopy practitioners’ have similar learning curves for all other bronchoscopic procedures, creating a need for this educational activity.

To address these gaps, this educational activity will provide attendees the opportunity to have hand-on practice under the guidance of experts in the use of EBUS-TBNA, EMN, robotic and electromagnetic navigation bronchoscopy, and airway management. Nurses, respiratory therapists and nursing/respiratory technicians will also have dedicated hands-on practice under expert guidance.

• Demonstrate techniques of convex probe endobronchial ultrasound
• Use imaging to depict mediastinal anatomy and lymph nodes

• Describe the roles of transbronchial cryobiopsies
• Describe the roles of bronchoscopy in lung cancer diagnosis and staging
• Describe the importance of biomarkers during the diagnosis and staging of non-small cell lung cancer
• Apply the fundamentals of starting a lung cancer screening program
• Describe the role of CT-guided and bronchoscopy for the evaluation of peripheral lung nodules
• Describe the general approach to lung nodule
• Demonstrate techniques for set up and assistance during the following procedures:
o convex probe endobronchial ultrasound
o robot assisted bronchoscopy
o convex probe endobronchial ultrasound
o during electromagnetic navigation bronchoscopy
o cryo biopsies (trans-bronchial for ILD and lung nodules, and EBUS assisted)
• Demonstrate techniques for rapid onsite cytology evaluation slide preparation during advanced bronchoscopic techniques
• Demonstrate techniques for set up and assistance during the following procedures:
o rigid bronchoscopy
o endobronchial stent placement
o endobronchial “hot ablative” modalities (APC, laser, electrocautery)
o cryospray therapy and cryo probe during endobronchial interventions
o endobronchial valves placement
o endobronchial balloon use and endobronchial blockers placement
